How do I add or remove a co-applicant to/from my account?

To add a co-applicant, please fill out the Amended Distributor Application (this form can be found in your eVO under the Communications tab, Library) and send the completed form to compliance@lifevantage.com.

To remove an existing co-applicant on your account, no amended application is required. Simply have the co-applicant send a letter to Compliance, with their physical signature, stating their request. It is also acceptable to take a picture of this letter and email it to Compliance as well.
